WAIVER OF LIABILITY AND ASSUMPTION OF RISK

PLEASE READ CAREFULLY

I understand that my and my family’s participation in any Live Love event, the organization, its programs, operations, and/or during 2025 is a voluntary activity, and that I am participating by my own free choice. In consideration of being allowed to participate in these activities, I hereby agree to assume the risks of property damage, injury, illness or death in any way associated with my participation in this activity. I agree to release, defend, indemnify, and hold harmless the Live Love Organization, its officials, employees, representatives, volunteers, and agents for any and all rights and claims for damages, including attorney fees, I now, or may hereafter have, and arising from or in any way connected with my participation in the Organization’s activities and programs. I agree that the terms stated herein shall also serve as a waiver of liability and assumption of risk for my heirs, estate, executor, administrator, and assignees and for all members of my family. In addition, I understand that for Live Love programs that are field trips, Live Love will transport my child to and from the location.

PHOTO RELEASE

Furthermore, I give my permission to have photos and/or video recordings taken of me or my child(ren) for publicity purposes during Live Love Organization’s activities even though we will not receive compensation of any kind for appearing in such photos or video recordings.

CAUTION

I acknowledge that I have carefully read this WAIVER OF LIABILITY AND ASSUMPTION OF RISK and fully understand that I am waiving any right that I may now or hereafter have to bring a legal action to assert any claim against the Live Love Organization in connection with my or my child’s participation in these voluntary activities and programs.

By signing the WAIVER OF LIABILITY AND ASSUMPTION OF RISK on behalf of minor(s) listed, the parent or guardian is agreeing to be bound by the above conditions on behalf of him or herself and on behalf of the participating minor(s).

Program Participation Agreement

Please read the statements below and check off the agreements prior to submitting your child's waiver

I am responsible for dropping off and picking up my elementary age student from program time.

Elementary students will only be released from the program to parent(s) or other authorized adult.

Junior High and High School age students are able to check themselves in or out during program time. Students are responsible for communicating their location to their parents.

In case of an emergency or if your child needs to be removed from the program, we will contact the parent immediately.

Registration and Facility Presence

Students must be registered with Live Love to come in the facility.

Phone rules

During program free time, students are welcome to be on their phone, please keep it appropriate and use headphones if volume is up,

For safety and respect to instructors, during organized activities we need your phone to be stored away. If it gets in the way, student will need put it away at front desk or step away from the activity to get focus.

I have discussed the Live Love Culture, Phone Rules, and Facility Presence Rules with my minor child(ren).
